Here’s everything you need to know about our Diabetes Education Class:

Summary

Who to refer:

  • Patients with diabetes mellitus who have either been recently diagnosed or could benefit from additional education regarding the disease and how to manage it.
  • After patients attend one class, they may be eligible to receive a prescription from EACN for a free glucometer, test strips, and lancets from us.

When:

  • 5pm-6pm on the 2nd and 4th Thursday of each month

Where:

  • Main Street clinic

How to refer:

  • Walk-ins welcome
  • Can also make appointments by either creating a “Diabetes Ed. Course Interest/Call” encounter on Practice Fusion (see below for directions) or by calling the EACN Pharmacy number at 352-474-2671.

Who: Pharmacy Students – Natalie Brumwell, Natalie Ponce, Deniella Chin-Quee

When: 5:00 – 6:00 pm on 2nd and 4th Tuesday of every month

Where: UF Main St. Clinic 1707 N Main St.

Patient Eligibility and Referral

  • Any person (does not have to be EACN patient) is welcome to attend the classes.
  • Patients are referred by:
    • Volunteers at EACN.
    • Diabetes education officers calling patients in Practice Fusion who have been prescribed metformin or other oral/injectable diabetes medications.
    • Announcements made in the Main St. Clinic waiting room for walk-ins.

How appointments are made

  • By phone to EAC Pharmacy number: (352) 474-2671
  • By Practice Fusion:
    • Open a “Diabetes Ed. Course Interest/Call” encounter for patient.
    • Write “Interested in attending [date] class” in Chief Complaint section.
    • Notify Diabetes Ed. officers of those interested in classes (use PRN numbers).
    • Patient will be contacted prior to the next class.
  • Walk-ins are welcome.

  • Each course lasts for 1 hour.

    • General risk factors
    • Signs/symptoms
    • Differences between the various types of diabetes
    • General treatment and prevention information
    • Complications
  • Information given on the 4th Thursday of the Month:
    • Nutrition and Healthy Lifestyle
    • Reading nutrition facts panel
    • Different food groups and healthy options to choose from
    • Exercise recommendations
    • Online ADA resources
    • Q&A session
  • After patients attend one class, they may be eligible to receive a prescription from EACN for a free glucometer, test strips, and lancets from us.
