Clinics Temporarily Closed
We regret that we have to temporarily close our clinics in the best interest of our patients and staff.
We are working tirelessly to be able to reopen and anticipate that will be able to reopen in late June.
We are excited to offer telemedicine appointments (either via phone or video chat) to our returning primary care and physical therapy patients for continued management of their conditions.

Returning primary care and/or physical therapy patients can request a virtual follow up appointment using this link:
- Please provide: 1) your name, 2) phone number, and 3) days and times when you are available for us to contact you by phone to arrange a virtual appointment.
We are able to discuss medication refills and similar issues over the phone. We recommend visiting one of our partner clinics if you have a new problem. For emergencies, please go to the emergency room or call 911.
